From Black, Indigenous and other People of Color to low-income and rural communities, we believe everyone deserves the opportunity to not only survive, but thrive.

  • Health is not only achieved through clinical metrics. We are building an organization that seeks to deliver greater wealth and power to local communities through our health-centered programming.

  • We’re a small nimble team with an activated network of partners. While we’re getting started with real communities across the country, there’s much more around the corner.

Meet the Team

  • Jeremy Liu

    As a nationally-recognized community development innovation expert, Jeremy uses his track record of transformative projects in real estate, community development, urban agriculture, food retail, green infrastructure, and arts and culture. Jeremy has worked as a real estate developer, business builder, artist and cultural producer, community organizer, and technology designer.

  • Sameer Sood (SS)

    SS is an engineer and physician who has created several organizations at the nexus of healthcare and design. As a systems think, he’s sought to engage stakeholders in understanding how societal structures, individuals behaviors, psychology and socioeconomic status impact a person’s health and pursuit of meaning.

  • Josh Prasad

    Josh is a strategist who is working on tearing down the structures that prevent an equitable society. This has taken him across government, politics, big consulting, non-profits, payors, health tech and direct care delivery where he has led and advised national leaders, campaigns and programs.
