Housing is expensive, and landlords don’t want to work with low-income tenants. We’re on a mission to change that. We build relationships with local real estate to Master Lease, acquire and develop housing units that can be leveraged for social care programming.

Healthcare is rapidly changing, and finding ways to integrate social care. We’ve designed models of social care that can integrate with health plans and hospitals and enable risk-based payment models that result in more money where it matters: the community.
