Fully Managed SDoH

Fully Managed SDoH rethinks health/social care integration. We work with community based non-profit organizations (CBOs) and organize a local coalition to support and coordinate these organizations as they address growing social deprivation. As we do this, we help negotiate contracts from local Managed Medicaid Organizations, Hospitals or other healthcare payers to drive new action on the Social Drivers of Health.

CBO Strategy and Technical Assistance

If you’re a community leader or organization that needs help exploring how to work with healthcare, like Medicaid, or other systems to structurally address aspects of social deprivation in your community (like homelessness, food insecurity, or poverty), we’re here to help! We offer a pay-for-success, income based pricing model that slides based on the needs of your community.

Braided Co-operative

As we’ve worked with communities across the country, we’ve identified new needs to help sustainably restructure the way communities interact with government, corporations, and other key stakeholders. That’s why we’ve designed Braided, a technology cooperative that aims to organize CBOs into a network and link them through shared technology and contracting.
