FwdSlash, in partnership with ActivateCare and a coalition of community-based non-profits, is re-designing the future of Health and Social care. Instead of being built by siloes, we’re starting together with a network of expert social care providers, national integrators, and then bringing in the necessary resources from clinical care to make a robust community of it’s own, for stronger communities and better health.

We’re building Braided to enable Community-Based non-profits to deliver services in a human-centered platform that helps fulfill the needs of multiple funders, data sources, and more. These groups can join our cooperative, gaining access to technology that lowers costs, increases access to new revenue, and links them into a network for technical assistance and continuity.
